Dungeons & Dragons Brings Baldur’s Gate 3’s Nastiest Moment to the Tabletop
The core dilemma of Baldur’s Gate 3 is established right from the opening moments: an illithid parasite has been implanted behind your eye. Left unchecked, it will inevitably devour your customized protagonist’s mind, transforming you into a tentacled, purple monstrosity. Because most of your companions share this dreadful condition, finding a cure serves as the party’s primary motivation. However, as the journey progresses, the parasite bestows formidable psionic gifts, tempting adventurers to surrender to the metamorphosis. At certain junctures, the narrative even demands that either you or one of your allies undergo this profound transformation.
Capitalizing on the monumental acclaim of Baldur’s Gate 3, Wizards of the Coast is now adapting that very same Faustian bargain for tabletop Dungeons & Dragons.
A recently unveiled Unearthed Arcana playtest document introduces a series of “Underdark Options” for players. Alongside aberrant archetypes tailored for Rogues, Barbarians, and Wizards, the release features a standout character progression: the Path of Ceremorphosis.
“The Path of Ceremorphosis outlines how a character might gradually succumb to illithid evolution,” Wizards of the Coast shared in a companion post. “This progression is largely informed by Tav’s potential descent into Mind Flayerdom in Baldur’s Gate 3.” The designers also credited the enduring popularity of past villainous mechanics in Unearthed Arcana as inspiration for the system.
The progression begins at 4th level with the Tadpole Host feat, forging a mutual bond with the internal parasite and granting foundational psionics such as the Mind Sliver cantrip. As characters advance, they can select from three intermediate feats to expand their toolkit. Illithid Thrallmaker enhances Charisma or Intelligence while granting the ability to cast Charm Person—emulating psychological domination. Tadpole’s Safeguard offers reliable defense via the Shield spell, a boon for martial and spellcasting characters alike. Finally, Ulitharid’s Might enhances Strength and unlocks tentacle strikes, an unarmed combat option exceptionally well-suited for Monks and brawlers.
Upon reaching 12th level, adventurers can culminate their evolution with the Full Ceremorphosis feat. As the playtest notes, the character sprouts four facial tentacles, their skull elongates, and their skin takes on a glistening purple sheen. The transformation officially reclassifies the character as an Aberration, though the current wording implies base racial features remain intact.
This final evolutionary stage branches into distinct combat specializations. Spellcasters can access signature illithid spells like Detect Thoughts, Levitate, and the newly updated Mind Blast. Martial combatants can instead focus on Brain-Seeking Tentacles, an offensive ability that grapples targets while dealing psychic damage. The design team frames this as a choice between psionic mastery and direct cranial consumption. Should characters gain access to an additional feat slot later on, they can even adopt both paths.
This design creates immense synergy for archetypes already tied to psionics. With a dedicated Psion class slated for next year’s Dark Sun release, illithid builds could achieve unprecedented power. Existing 5th Edition subclasses from Tasha’s Cauldron of Everything—including the Psi Warrior Fighter, Soulknife Rogue, and Aberrant Mind Sorcerer—already rely on Psionic Energy dice, making them natural fits for this parasitic progression once adapted to the revised ruleset.
Ultimately, this playtest offers video game fans a direct avenue to replicate their Baldur’s Gate 3 builds at the physical game table. As with all Unearthed Arcana material, these rules remain subject to adjustment based on community feedback, with official surveys launching on August 27. The document notes that these subclasses and feats may appear in future sourcebooks, hinting that a dedicated Underdark release could soon be on the horizon.
